.pdf upon registration

I am looking for someone to make me a php snippet and walk me through where to put it so when my members register, they have to fill out a 1 page pdf that I have, click the SUBMIT button and it e-mails to the e-mail we provide. The e-mail button and the whole pdf is created.

I basically want it to just load a pdf page, they fill it out, and then click next and then they fill out the registration form for the forums itself.
