Collect demographic info on the payments.php

I have a site that used to use a form for people to register to join - it was collecting demographic info and directing them to paypal for payment
Right now we have redone the form to point to payments.php rather than paypal BUT we would like to integrate further
I would love to be able to combine the form into the payments.php so that collecting in the demo info and subscribing are seamless
This kind of programing is beyond me

Anyone have any ideas ?
