There are actually very many like this available in the mods section.

However, I have yet to find one with the following features:

-Works with usergroups so you can limit guests, etc from using it.
-Has a catchpa (for anti-bot usage)
-Has some kind of spam protection... like for example, only 1 per day or whatever.
-Has a pre-set message instead of a box for users to type in thier own message, which they can use to trick people into thinking they are staff and email someone asking for thier password, or some kind of spam abuse. Possibly have a box where the user can add a personal note at the end of the pre-defined message, but that's it.

If anyone finds one like this, let me know as well...
