Problem with viewed/unviewed threads

One of my forum members complains as follows:

There seems to be an issue with showing which threads I've viewed. Sometimes threads that I haven't opened yet will not be highlighted (as though I've viewed them already) and sometimes threads I've seen the most recent posts in will stay highlighted as though they contain new posts.

What's going on and what should I do?
