Adding Mutli. moderators with changing permissions every time

I want to change the "Add New Moderator to Forum" template. There are 6 things I change every time I add a moderator to a forum and would like to make those changes permanent so I don't have to change the option from Yes to No every time.

Any ideas what I should edit? I thought it was "moderator.php" but I don't see how to change the options from Yes to No or vise-versa.
