Is there a standard recommendation for authors to document their work as they go?

If nothing else, comments are handy when going back over troubleshooting later. I suppose I can just drop in a <!-- [hack-or-modue-name] --> code to be modified or added <!-- [hack-or-modue-name] --> as a minimum Any suggestions or best practices? |
