Fatal error after upgrading

Hey guys,
Upgrading from 4.0.0 and getting the following error after upgrading/logging into Admin CP:


Warning: require_once([path]/dbtech/username_change/includes/hooks/cache_templates.php) [function.require-once]: failed to open stream: No such file or directory in [path]/includes/class_bootstrap.php(1419) : eval()'d code on line 1

Made a clean install in a new directory using a database backup - any ideas?

Figured it out with disabled hook but now my CSS appears to now be working and the site looks dreadful.. any ideas?
