Hack needed, please?

Hey guys I come to all you wise-minded people for a request =)

What I need is this:

For my forums I have a Site Check place ok? I need it so that if you go to reply to a guys request to get his site checked, there will only be the Template for site checks which must be filled in, not the reg. reply box and stuff.

Is this possible?

If you are going to do it I can give you the fields I need and all the details. Thanks!
