And I think this time I deleted something more than the spam link from the index file. What am I missing at the top of the code? I'm really not a tech at all, and appreciate all help!

I think the top should look like this:

<?php
/*================================================= =====================*\
|| ################################################## ################## ||

I guess because the hacker inserted a link at the top of index.php, and when the OP deleted it, he or she accidentally deleted a few too many characters.
