I'm surprised nobody has done this as a mod yet, but I'd really like to replace the vBulletin 4 reCaptcha anti-spam system with Microsoft's ASIRRA as it seems to be a lot more effective.

I know about that one but it's not been updated to vB4 and the author has vanished. Not sure how to update it myself.
