Modify Thread Meta Tags And Descriptions

How can you edit the meta tag and descriptions for individual threads?

The only possibility I know is to edit the keywords which are stored in the vb_thread table in each thread under keywords.
vB creates a meta-tag for each thread based on those keywords, e.g.:

<meta name="keywords" content="avatar,wunschliste,manuell,titel" />

Thanks Adan0s-- I am not familiar with how to do this--could you explain this a bit more?? And also if anyone else knows of a mod or plugin to do this or any other way I would love to hear it!!
