How can i add a bbcode set with popup menu like when you click the image bbcode (shown at below pic.)

What i would like to do is, there will be an icon on editor. And when user click it, a popup will appear and ask "whats your name?". after user fill it, another popup will appear "how old are you?" etc...
Is that possible ?
