Custom user fields to the send message form

Hi,

How do I add custom user fields to the send message form ?

"sendmessage.php"

if it possible, i want put to all <input text> for:
- Country
- Zip Code
- Phone / Fax
an other...

Only this to add <input type>

Then, edit file variables and conditions in their files php.
