For some reason one of our Forum Moderators was changed to a Super Moderator. None of the Admins or Mods made the change and it did not show up in the the Control Panel log. Anyone know what could have happened?

Negative. Does not have Admin access to the CP.

It still could've been done without it showing in the control panel log

You can change the database using a query so that they'll be in the super moderator group.
This can be done in the admincp under maintenance or with phpmyadmin

In your control panel log look for anyone using query.php
As for phpmyadmin, im not sure

Was his Primary Usergroup set to Super mod or was it an Additional Usergroup???

do you have the vBplaza hack installed as unless you turn it off users can buy access to usergroups through that. Happened to me when one of my members did it, but they came to me straight away and told me, so no harm was done.
