Easily create forms with no HTML or PHP knowledge. Questions can be made compulsory and are checked before the form is submitted. Forms can be previewed. (You can create multiple forms)

There are two posts full of screenshots of this modificationInstallation
  1. Download and then extract .zip archive
  2. Upload the contents of upload folder to forum root
    (Allow Overwrite "YES" for overwrite)
    clientscript\easyforms.css -> \clientscript\
    clientscript\easyforms.js -> \clientscript\
  3. Import product file (product-easyforms v4.0.xml) in admincp
    AdminCP -> Plugins & Products -> Manage Products -> [Add/Import Product]
  4. Change usergroups permissions at:
    AdminCP -> vBulletin Options -> Easy Form Options
Question Types:
  • Single line text
  • Multi-line text
  • Multi-select checkboxes
  • Single-select radio buttons
  • Single Select Dropdown
  • Yes/No
  • Multi-line text field using vbeditor
  • Human verification question
  • Custom Question (can query database or do whatever you want using php and html)

Form can Submit to:
  • new thread in forum
  • new post in thread
  • new pm to user
  • email address
  • save to database
More Features:
  • Form list categories (optional)
  • Preview form (can disable for individual forms)

  • Prefix support - you can choose which prefix you want thread to have
  • Different thread/pm title to form title with variables {username} {userid} and {q_1} (replacing 1 with appropriate question number)
  • Poll Options: Multiple choice, public vote, poll question, poll options, poll timeout
  • Regular expression match for single line text input field
  • Usergroup permissions per form
  • Forumid/threadid/username can be specified by input via {value=X} in dropdown, single line input or radio buttons
  • Multiple dropdown select for form usergroup permissions (still optional - leave blank to not use)

  • Import/ Export your forms as XML files - great for transferring forms across forums or creating backups

Is there anyway to make one form lead to another ? I am thinking of a basic workflow.

User submits form and becomes a new thread in a subgroup, admin opens it and after some checks can click another button somewhere which will grab the information n that form to populate another form that will do things like add a user to a group, move a thread to a different forum and send a pm to the original user.

I can get everything working (I think) using two seperate forms but I'd love to tie them together so the admin doesn't have to cut and paster from one submitted form to another open form.

I have searched this thread and can't find the answer.

I have a simple form that is emailed to me when submitted.

I really need to send a copy of the submitted form by email to the person submitting it.

How would I send an email to the forum member who submitted the form?

I looked again though the posts and found this;

But where in the form do I put this????

Haven't used forms since upgrading to 4.2.0.

Just started using them again and I've found that it's dumped all the permission settings from the AdminCp-->Options-->Settings-->Easy Forms and won't save them when they're reset!

Anyone experienced this and anyone know a fix?

Thanks for any advice or help.

EDIT: Problem solved: for some reason, would not save settings when accessed with "show all settings" but would save permissions fine when the individual Easy Forms option was chosen from the list.

There's a Way To Upload Files (Attachment) Directly In To The From ?
(Without Using A Thread)
Maybe By Using A Custom Question ?

Is there any way to allow a user to upload/manage attachments to a previously submitted form, if they are able to edit previously submitted records?

edit: I guess it's a somewhat similar question to the post above me.

I am having a problem where I press create a form, press "save" and then on the next screen modify something, hit "save" again and it redirects me to the forum rules. So stumped, anyone got any idea?

is this possible i want my forum private so permission are set so unregistered users cant see forum .

but i would like to use this as an app to enter if approved i send them a invite ive added the app link to navbar and added permissions but still no access

Very useful AddOn! Thank you! Only question I have is: Is it possible to add my custom human verification? e.g. Motion Capcha? How would I do this?

Note: Many of the bugs mentioned above, e.g., the "navigate away from this page" popup, are cause by using the ckEditor in your form. In particular, note that you can NOT have more that one editor field on a form or the form will not submit.

Workaround: Change any ckeditor items to multline text fields.
