Custom Title Undername Editable by Mods/Smods/Admin

Hello Fellow People,

I was wondering if someone could re-direct me to the hack that's like:

[UserName]
[Custom Title by Usergroup (editable by mods/smods/admins)]
[custom user title]

I tried Jens, but its by Usergroup, and mods/admins can't personalize specificly.

I'm not sure that's the one i want, here, I'll give you a better example.

Look at the picture, on their custom titles are 'serpensortia' and 'just call me 'snuffles'' yet, above their name is a custom status, and both of them are super moderators, so i don't think it's the mod from Velocd, any ideas>
