View Attached Images Inline - Yes
Enable Thumbnails - No

Did you rebuild them in the maintenance section under update counters?

What version are you running? I think this was a bug in one version.

What version of PHP? And you should probably upgrade to 4.2.2.

Are you using gd2 or imagemagik? Did you check settings?
