Looking for a mod similar to what vB.org is using in the modification area that allows coders to add there details in a predefined form. Been looking all day and haven't found it yet. I know Ive seen similar mods in the past so not sure why i cant find one now.

Chief First Post has a similar template in there. You could probably use that. There is also a Modification mod that is like what vb.org uses.
