Instead of 1 'Remember Me' box, I am seeing 3 of them.

How can I remove 2 of them?

Where are they located?, if its right by each other there in the navbar template

Look in your navbar template and serch for "cb_cookieuser_navbar" you have 3 of them...delete the last two.
