You can use the "Form Hack" to have multiple forms, any questions you want, email/pm/post application details, etc. I started using it to recruit CS:S Server Administrators for my group. I plan on making another form for general community staff. It's easy to customize; just takes a little while to get used to.
I too support this mod. It's very useful - for members wanting to join a guild via answering the questions on the form you custom make.
