Extra registration/profile fields ..... after registration

Im also in need of something like this for 3.07 or 3.5

we have a special usergroup for "street team" members and we need to have them enter their mailing address when requesting to join the "street team" forum. We need their address because we send out posters,stickers and other advertising materials to every person who is granted "street team status".

anyone know of a hack or place to have this custom made? We will pay very well to have this made.
