Membership data management and subscriptions

I am trying to find a solution for an organization that is currently running a vBulletin forum and would like to use vBulletin to help implement local chapters and individual membership data. We have some good ideas on how to get subscriptions to do most of what we need, however, we're running into an issue with how to get some extra data from users when they purchase a membership to the organization (via subscription).

We need some info such as phone number, address, and what chapter they wish to be a part of (if any), and this data needs to be stored in a database (can be a custom table if necessary).

I know you can have user fields for these items, but I imagine most members won't fill that data out upon registering for the forum, and we don't want to require it for registration on the forum. We only want to require it for members of the organization (you can be a member of the forum without being a paid member of the organization).

So is there any way to add a form to the subscription that gets this extra data upon purchase? If not, how difficult would it to be to make a mod that would do what we need (I am a programmer)? Or, even better, is there a mod already out there that does this?

I would just use the User Profile Fields and leave it up to them to fill in the necessary information. Chances are if they're paying money, they'll fill out the information.

Include some text in the subscription page to indicate they should edit their details in the User Control Panel to fill out these fields.
