Code to replace log-in for forum

I thought this was repaired but it isn't. I had someone try to place a L column on the front page and they moved the sign-in to the left column but it only shows up periodically. The L column was never completed. So, when users go to log in, most of they time they need to try to post in order to log in.

How do I replace the original 'log in box' code into the bb again? I'd like to place it back into it's original place right above the Nav bar where it says quick links and log-out with all of the following items:

User Name
Password
Remember me
Log in

Is there a code I use and where would I find it? Also, where do I place it?
