Banned: False?

Every now and then (almost often) I get members complaining that they get this message (see attached)

The truth is that they were not banned (account nor IP). I am trying to trace what is causing this false message. The forum they are accessing is open to everyone as well, permissions not modifiied and the usergroup is valid without any settings to cause this false alarm.

I've also attached my entire list of everything installed (hacks, extensions, plugins, etc)

Any help would be appreciated. Thank you
