Creating a Shop Location Rating forum

I am in the process of creating a form (using Bananalive's form mod) that will allow my members to provide information about a Hobby Shop they go to / know about. The format of the output is already set to go (I have a few items to modify - adding email, adding output images, etc) as well as creating a new forum post for each shop entry.

What I am looking to know is if there is a mod that exist that would modify the output to include store rating and alert the owner of the store (if email is provided) when some posts a review of their store?

Since the owner of the shop may not be entering the data, having the general reply to thread message is not going to work in all cases.

Not sure if something like this would work OR maybe there is an alternative solution for contacting the owner/shop contact (I can live without the 1-5 star rating system).
