A database backup will only backup your database (usually all your data). If you have files you uploaded for a modification, then those will not be included in your database backup. If you have attachments or avatars stored in the filesystem, then those will not be included in the database backup. Only the database is backed up in a database backup, not the files.
You may want to ask your host if there is an option in your server control panel that will allow you to easily do database backups and/or file backups.
