Want to create ID card

Hello everyone.. I run the largest VW & Audi enthusiasts group in the Denver Metro area. I was curious if there is a way to create a new php template page that can harvest certain database information and populate that information into an Identification card. I would like to have my club logo on it, their real name; first and last, screen name and rank, join date and a date that is 6-months after the current date for an expiration date.

I can create an image file that can contain the logo and some standard information, but was wondering if anyone can help me create the rest.
