Log in problems 3.8.3

Ok I have had it up and running about a year Now.
The problem I'm having is some of my members can't log in I reset the pass word and they still can't log in. Keeps saying wrong user name or pass 1 out of 5 trys. And new members are having the same problem but what's odd is some time new accounts will not work and then some on will make one and it will . Any one know what's up do I have to do a upgrade ?
