Spam and spammers: How do you battle it?

I have noticed a surge in spammers lately, and they are getting more creative. Image verification is being handled, another little thing I use where you pick custom questions - that is being passed. They are even using avatars and other customizations to make the account appear real... Then a couple of days later - boom, tons of junk posts.

Cleaning them up is easy thanks to that script (one touch ban and clean - hopefully someone continues the development for it).

My question for those out there are how do you battle spammers? What ways have you found to be most effective? I have seen random posts where this question is partially covered. Just wanted to survey the community.
