Taking a skill test ~ code needed ~ star rating

I'm looking into having a basic Skill Test on my forum. Not as a requirement to post or anything like that but more for fun.

I'm looking for something that I can add my own questions and correct answers to then when submitted it rates the member from 1 to 5 stars depending on how then did.

Anyone have a clue where I can find this?

Also would be cool to add that members score to their profile but thats just a thought.
