How to setup custom fields?

I've stumbled on a forum that uses vbulletin, and I am liking how they have this thing setup:

I wonder if anyone out there knows where I can find these extra fields to put into the reply box, and have it appear on the forum list on the thread.

Anyone know? I don't exactly know how to name this... mod of sort.
