Needed: Users Delete Own Account

AlphaWolf has given permission for someone to port this hack to version 3.5. I think it should be fairly simple. I just need a form that will do the work.

I have a board with over 15,000 users and it would be nice to let them delete themselves when they graduate from our community.

Any takers?
