I use Vbulletin 2.0.1 and I wonder if any of you can tell me, how it is that I can create something similar to the link below. It appears to be a form, but my limitied knowledge of HTML, and honestly, MB software directs me here to ask if any of you know how I can create a duplicate of the form you see when following the link below.

This link is from a online game and is used by members of a guild, to list their character's profile, and would be very useful to me.

If anyone can please assist, I would be quite grateful.
