Old Modification Question - Admin/Mod Comments

I remember from 3.6.x that there used to be a mod that had admin/moderator comment box above a thread. It was useful for a lot of things, but I saw it used primarily for applications or reasons for modding a thread.

For the life of me I cannot find it, and I am looking to see if it has been updated for 4.x. I tried some search criteria but did not find what I'm looking for.

Does anyone remember this mod?
