World of Warcraft Application Form

Hey people

Ive been searcing alot recently, after an uppdated mmo / wow application forum for VB 4, but it seems the makers of the previous version aint to keen on uppdating it or dont have the time.

So im wondering, anyone who can make something like this for vb 4?

Thats the basic layout

Basically its a form witch let ppl easy make application to guild or clan whatever you want to call it.
When ppl fill out the forum and click Send it will be automatic posted to a forum ( witch the one who apply cant see ) so it will be a sendt to a forum where only a certain usergrp can see.

It need to be customize able enough so admins can change questions and add more question if they need

Would also be great with and option to make a poll when someone send in the application form.

Not sure if someone can make this, and im sure many would love something like this without having to downgrade theyr forum to be able to use it.
