Is there a mod or hack for this?

I belong to a online gaming clan and I am trying to get a "Match Report" to post results on our forums. It is a form that we use to put in the details of the matchs we play. I made it on DreamWeaver 8.

Am I missing something? It won't post the results, just goes to a blank page. Any help or ideas on this would be greatly appreciated. Is there a mod or hack to allow custom made forms to be posted on vB?
