VB3 Forms?

Here is an example... Hypothetically, I run a business. Instead of using the Post new thread, I'd like to post a new form. So, when I click on, "Post New Thread", a small form such as this will come up:

Make
Model
Vin
Title

In which I can make mandatory fields.

Upload photo's. (this would be the same is "Manage Attachments"

Then I would like for the results of that form to be displayed the same way a thread would be displayed.

The form would be customizable of course, and fields would be limitless.

Has a hack similar to this been made yet?
