Integrate a survey into the signup process?

Hey guys!

I'm looking to see if anyone can point me in the right direction towards fulfilling a need in my vB community .

Objective:

When someone signs up on my board I want them to fill out a survey which includes information about them and their reasons for joining. I want this survey to then be posted up on a moderators board for review and activation.

I'd like to be able to customize the look of the survey and have it as a seperate part of the registration process.

For example -

They register and they now have an inactive account. They can read posts, etc. There is now a message at the top of the page that welcomes them and invites them to take the survey to have their account activated.

Then then take the survey, its posted in the mod board (it can be emailed) and then there account is ready to activate.

Alternative

The alternative is to give them access to a board where they can post their survey answers . The trick with that is I want to try and eliminate the need to copy/paste the 15+ questions into the board. I want them to have a template to fill out.


Comments, suggestions? Any ideas on how I can make this happen? Is there something like this already out there?
