warning hack help

ive had the warning hack installed and when ever i try to warn a member i get the following error:

Database error in vBulletin 3.0.3:

Invalid SQL: insert into pmtext values ('','1','admin','You have been warned','You have been warned for the post: Your Post. Your warning level is now: 1. This action was taken for the following reason: stop spamming twat!!!. Please reply back if you have a dispute. admin','a:1:{i:166;s:12:\"test-account\";}','4','1090250859','1','1')
mysql error: Column count doesn't match value count at row 1

mysql error number: 1136
