Blank Attachment in Advance Reply Option

Hello guys

I upgraded my forum last night to 4.2.0

Last night i faced some problems aswell but sorted them out....

Now what i am facing is that in Advance reply option when i click on attachment it just comes blank

I tried disabling Plugins but still same problem...

Any had any problem like this??
