How to get these features?

I'd like to have the following features on my forum:

1) How do I make image usernames (as shown in screenshot above)?

2) How do I add custom rank/badges under a member's username?

3) Can anybody link me to a good rep mod?

4) How do I display the Members/Guests viewing a board, and a thread?

5) How do I add custom BBcodes to the quick reply box?
