Help! Stopping spam - cant get plugins to work

Hi

I've tried installing a couple of the anti spam registration tools so you can add a random question in the user registration process like this mod:

Problem is when I go to test it I cant see the random question appear in the registration form? I've tried disabling the cuttent captcha image verification but no joy.

Any ideas?
