Custom membership fields + Google map questions.

-Can anyone tell me how to make these fields show up on my member list, using this VB as an example? I have the admin enabled and the default ones show up on my member list, but not the custom ones like this guy does.

-Also, how do I put one of those membership maps/Google maps on where members can put their locations on a pin on the map?
