How are mods done now?

Got sidetracked on other issues about the time of all of the "hook", AJAX and plugins craze took off about 2 years ago.

So would like to know: where's the RTFM on how to install these new fangled thingies.

Noticed some hand coding is still involved (something recognizeable!), but don't understand the new term of "install as usual" with .xml. Like where do you install it? :erm:
