Threads are sitting above the rest of forum

Please can someone help me here.

I installed the 'thread Prefix' hack, this didnt work, so i then removed all the template edits and all the php edits.

The problem is am now left with this.

I can seem to work out why this has happened and how to get it back to the proper layout.

Please, please help as this has really got me confused.
