Adding custom fields to threads

I need someone to help me add custom fields to each of the threads in my vBulletin. Specifically, for each thread, I'd like the thread to have separate fields for: address, city, state, zip, phone, and GPS coordinates. Thus, when a person starts a thread, it'll have a title (which I need renamed to "Name"), and will also have the above referenced fields available as optional fields to fill in

I need the customization done fairly quickly if anyone is interested (might be willing to pay a little $$ as well ).
