Problem trying to merge Login/PM info into Header.

[ISeemToBeAVerb]

I'm trying to add the Login/PM info box to the right side of my logo in the header. The script works great when I get rid of the Login, PM Info code and just leave the Login Form code. What am I doing wrong here?
