Need to place some ads down side of index

One of my clients wishes to place several ads down the left side of his index page and his forumview pages. I thought that the ultimate side columns would work great for this, but it doesn't place content where he's wanting the ads at (and it doesn't output the JS)

So I basically need help with 2 things:

1 - What is the conditional to place in the header template to only show the code on FORUMHOME and forumview pages?

2 - Where do I place the ad code in relation to the other code to make it appear like he wants it? All of the HTML I've tried only kind of looks like his example...

Any help is appreciated (design = obviously not my thing)
