EASY FORMS FOR vB 4.x here


Easily create forms with no HTML or PHP knowledge. Questions can be made compulsory and are checked before the form is submitted. Forms can be previewed. (You can create multiple forms)

Backwards compatible with vb v3.7.x and 3.6.8

Example Form | Help File | Version History

Question Types:
  • Single line text
  • Multi-line text
  • Multi-select checkboxes
  • Single-select radio buttons
  • Single Select Dropdown
  • Yes/No
  • Multi-line text field using vbeditor (max one per form - limited by vB)
  • Human verification question
  • Custom Question (can query database or do whatever you want)
  • Date (month/ day/ year fields)
Form can Submit to:
  • new thread in forum
  • new post in thread
  • new pm to user
  • email address
  • save to database
Installation:More Features:
  • Form list categories (optional)
  • Preview form (can disable for individual forms)
  • Optional questions
  • Form Sections
  • Prefix support - you can choose which prefix you want thread to have
  • Different thread/pm title to form title with variables {username} {userid} and {q_1} (replacing 1 with appropriate question number)
  • Poll Options: Multiple choice, public vote, poll question, poll options, poll timeout
  • Regular expression match for single line text input field
  • Usergroup permissions per form
  • Forumid/threadid/username can be specified by input via {value=X} in dropdown, single line input or radio buttons
  • Multiple dropdown select for form usergroup permissions (still optional - leave blank to not use)
  • Option to show link to form list in navbar and quick links (separate options)
  • Import/ Export your forms as XML files - great for transferring forms across forums or creating backups
  • Specify bbcode for questions/ answers/ sections for the form's output
  • Form Hooks: (uses php code)
    1. Before form submitted
      Useful for: your own form validation, multiple form outputs, etc.
    2. At form start
      Useful for: banning userids/usergroups from form, setting minimum post count,etc.
  • Attachments allowed for posting new thread
  • Mass Copy Forms & their questions

TWO Suggestions, If I may.

Or help to do them, rather.

1. I have added a field for an address. But when the address is entered and displayed on the forum, it shows as "ADDRESS:the first line of address
Then the second line of address here.

I would like that to be ADDRESS:
1st line of address here
and the rest in line like this
and this
etc.

2. How would I be able to Add a field where the "User" can enter a picture? by just adding their url to the pic Please?
(similar to vB with the function of this feature on the top of the reply text area... )

The above two would be the icing on the cake of a fab product.

1. Edit Form Add "\r\n" to end of Question BBcode

1. \r\n are already there Mate. (If I add another one to it, it will be the same space on all answers - whereas I would just like it for the Address field).

2. What exactly do I need to do Please? What do I enter in there etc? As you can tell, my experienced is very limited.

no ...it is not fixed ... i disabled slideshow (which takes pictures from one thread) and now forms works and there is no picture on formslist.

i disable plugin for slideshow ...

I just want to say great mod btw and thank you! I just have a small issue and I'm sure this has already been answer but I can't seem to find an answer in the thread.

I'm using the Email Form only because I want to go to our support group. The form and data comes through fine but the format is just one big block of text.

Example (current format):

I want the email out put to follow the same format as the online form

Example of what I'd like:

First - very cool mod. It will make a difference on our forums. Thank you for the work.

Second - VERY COOL MOD!

Third - I am a newb - question: trying to use custom output from a radio button question with three options. When I format as follows:

Type: {q_2}
The output is:

(pulls the question number vs. the radio button selection)

Any ideas????

This mod will be great, for a site, but the link in your psot abotu creating/viewing forms (http://your-forum.com/forums/misc.php?do=forms) does not work
