Captcha & Registration - How to put?

hello guys,
I'm having my own website. I Just want to ask that i want to enable

* Captcha System while member registers
* Want to put 1 question & Answer should be NO while registering
* Want to put 1 question & Answer should be MY WEBSITE NAME while registering

How to put it please help me ASAP?

Right now my captcha doesn't show anything. Many people telling me that they cant register cuz of captcha
